Implementation, however, relies on frontline staff's limited tacit knowledge amid an evolving service landscape.
Care Connect AI overcomes this with "Agentic AI" that reasons over complex narratives, navigates a Scottish services knowledge graph, and serves as a "Loneliness Companion" and service broker, surpassing static directories and basic chatbots.
